Creating a time off request form is essential for efficient leave management. Follow the process below to easily create one using Google Forms.
Navigate to the "Template Gallery" and select a "Blank" form to start creating your custom time off request form.
In the "Untitled form" field at the top, type "Time Off Request Form" to give your form a clear and specific title.
Below the title, add a brief description such as "Fill out this form to request time off from work" to clarify the form's intention to users.
Add questions like "Full Name", "Start Date", "End Date", and "Reason for Time Off". These questions ensure that you gather all the necessary information for the request.
Choose appropriate response types for each question:
"Short Answer" for name,
"Date" for start and end dates,
and "Paragraph" for the reason for time off.
Click on the three dots in the bottom right of each question box and select "Required" to ensure that respondents cannot skip these essential questions.
In the "Settings" tab, go to the "Presentation" tab and click the "Edit" link for the confirmation message.
Customize the message to say something like "Your time off request has been submitted successfully". Click "Save" to preserve your changes.
Click on the eye icon at the top to preview your form. Ensure that all questions appear as intended and that mandatory fields are set correctly.
Go to "Settings" and under the "Responses" tab, adjust preferences such as allowing response editing and limiting the number of responses.
Click on the "Send" button at the top right. Here, you have several options to share your form:
Email: Directly send the form to specific email addresses, along with a customized message.
Link: Generate a shareable link that you can distribute via chat, text, or social media.
Embed: Obtain an HTML code that allows you to embed the form directly into a website or intranet page, ensuring seamless integration.
