Step 1: Select the "Multiple choice grid" question type.

Click the "+" button on the right to add a new question, then select the "Multiple choice grid" from the list of question types.

Step 2: Set your Likert scale. 

In the "Rows" section, enter the statements or questions for your scale. In the "Columns" section, enter the scale points. For example:

• Strongly Disagree
• Disagree
• Neutral
• Agree
• Strongly Agree

Step 3: Toggle "Require a response in each row."

You can find this at the bottom of the question box. This is to make sure that the respondent doesn't skip any of the statements or questions. 

Step 4: Preview your form.
