If you haven't already, create a Google Form that fits your needs. This form will be the source of the data you want to send to Trello.
Go to Zapier and sign up for an account if you don't already have one, or log in.
In your Zapier dashboard, under the Create button, click on "Zaps".
Search for and select Google Forms. Zapier will prompt you to connect your Google account if it's not already linked. Follow the prompts to authorize Zapier to access your Google Forms.
Choose the trigger event as "New Form Response." This means your Zap will trigger every time a new response is submitted to the linked Google Sheet.
Go to the Account tab and connect your Google account.
In the Trigger tab, click on the dropdown menu and select the form you want to connect to Trello.
Search for and select Trello.
Choose the action you want from the dropdown menu on the side, under the App & Event tab. For this guide, we chose "Create Card."
Connect your Trello account in the Account tab by providing Zapier with the necessary permissions to access your Trello information.
Map fields from the Google Forms response in Google Sheets to the card’s title, description, labels, members, etc. For example, you could use the form response to set the card's name and description.
Before turning it on, make sure to test it to ensure that it works as expected. Submit a test entry to your Google Form and verify that a new card is created in Trello according to your specifications.
If the test is successful, go ahead and turn on the Zap by clicking the "Publish" button at the bottom. Your integration is now active, and new Google Form responses will automatically create Trello cards.
