Follow the steps below to create a Google Forms incident report template. 

1. Create a New Incident Report Form

Click on the "+" to create a new form. This will be the basis of your incident report template.

2. Set the Title and Description for Your Incident Report Form

At the top, where it says "Untitled form", enter the title of your form, such as "Incident Report Form". Below the title, add a brief description to explain the purpose of the form.

3. Add Questions to Collect Incident Details

Use the "+" button on the right side to add your first question. You'll need to collect information like the date, time, location of the incident, a description of what happened, and the reporter's contact information.

4. Select Appropriate Question Types for Each Inquiry

For each question you add, select the most suitable type from the dropdown menu on the right. Opt for 'Short answer' for quick responses, 'Paragraph' for detailed descriptions, and 'Date' for reporting dates.

5. Ensure Essential Questions Are Marked as Mandatory

Toggle the "Required" slider at the bottom right of each question to ensure respondents don't skip essential questions. This guarantees you gather all the necessary information.

6. Customize Your Form's Appearance with Themes or Images

Click on the palette icon in the upper right corner to change the form's color theme or add a header image. This customization can make the form more engaging and fit your organization's style.

7. Organize Long Forms into Sections for Easier Navigation

If the form is lengthy, consider organizing it into sections for easier navigation. Click on the two rectangles icon next to the "+" button to start a new section.

8. Preview the Form to Ensure It Meets Your Requirements

Before sharing your form, preview it by clicking the eye icon at the top. This lets you see the form from a respondent's viewpoint, ensuring everything looks right.

9. Share Your Form to Start Collecting Incident Reports

To collect responses, click the "Send" button at the top right. Decide whether to share your form via email, a link, or embedded on a website.

10. Review and Analyze Incident Reports Through Responses

Access the "Responses" tab at the top of your form to see what has been submitted. You can view the responses in Forms or export them to a Google Sheet for deeper analysis.
